Registration forms can be collected from reception or downloaded by clicking here. You will also need to make an appointment with the nurse for a new patient health check. When you arrive for your new patient health check you will need to bring your completed registration forms and a sample of urine with you. Sample bottles are available at reception.

After your health check you will be informed by the practice in writing whether you have been successful or not. This usually takes up to 5-10 working days. If you are accepted you will receive a medical card through the post with one of our doctors' names on.

You can then see whichever doctor you prefer. We do not discriminate on any basis and we expect the same level of tolerance from all our patients and visitors.

In common with most local practices our list is heavily oversubscribed, however if you ask at reception they will let you know the most up to date position.

Please note that you cannot yet register online. You can download the forms but will need to complete them and take them to reception.
