How to Register
We are happy to register new patients who live within our practice boundary area; which is M5 and M6 and M50. The practice has the right to refuse your registration if you live outside of these areas.
Register online
In order to undertake the registration process we require you to complete a Registration Form and also a New Patient Health Questionnaire.
Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
If your registration to join the practice has been successful, you will receive a welcome email from the practice within 7 working days.

The National Care Record Service (NCRS)
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
