Register with us as a New Patient

We are happy to register new patients who live within our practice boundary area; which is M5 and M6 and M50. The practice has the right to refuse your registration if you live outside of these areas. In order to undertake the registration process we require you to complete a Registration Form and also a New Patient Questionnaire. We ask that you complete the online form below to do this; alternatively please complete and hand in one of the paper copies. Upon receipt of your registration paper work , the practice will contact you within 3 working days to arrange a health check.

We also have a site dedicated to the staff and students of the University of Salford. Please ensure you use the correct registration links, as this may delay your registering with us.

Acceptance on to the list is as at the discretion of the Doctor. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.
